Your benefits are effective on your date of hire.
If you are a SIMNSA enrollee, your benefits are effective the first of the month following your date of hire.
• Yourself
• Your married spouse/domestic partner
• Your child(ren) up to age 26
• Employment ends
• You fail to make premium contributions
• You no longer meet the eligibility requirements
• The benefit plan terminates
You are eligible on the first day of the month following your 30th day of employment with the company.
